Insight Investigations was founded to give organisations access to truly expert, independent investigators who understand the complexity of the work from the inside. Our team has spent decades conducting investigations within the health, education and disability sectors — which means when you bring us in, you don't need to explain how your industry works.
We are based in New South Wales and work with organisations across NSW and Queensland.
Our investigators hold CAPI licences and relevant professional qualifications, and we work within the standards required by the Office of the Children's Guardian, the Fair Work Commission and other regulatory bodies.

Alex Tawfiq — Director
Alex founded Insight Investigations after spending 20 years conducting and managing investigations within NSW Health. In that time he developed a deep understanding of the health sector, the reportable conduct scheme and the standard of investigation required by the Office of the Children's Guardian. He is a non-practising solicitor and holds a CAPI Masters and Operators Licence. Alex has been recognised by the NSW Ombudsman as an expert in reportable conduct investigations and regularly assists organisations across NSW and Queensland with complex child protection and workplace matters.

Amanda Hadley — Principal Investigator
Amanda brings 14 years of experience from the Health Care Complaints Commission, where she conducted and managed complex investigations involving clinical practitioners and health system complaints. Her additional experience as a Senior External Relations Officer with the NSW Ministry of Health gives her an understanding of health policy and systemic issues that goes well beyond the individual investigation. Amanda is particularly experienced in matters involving sensitive evidence, bereaved families and situations where multiple regulatory processes are running concurrently.

Stacey Whitmore
Stacey brings a distinctive combination of law enforcement, child protection and workplace investigation experience built over a career spanning NSW Police, community services and care management leadership. During her eight years as a NSW Police Officer she worked as a Homicide Intelligence Analyst and specialist child protection investigator, giving her a strong foundation in evidence-based practice and managing sensitive, high-stakes matters. She now applies that expertise to complex misconduct and reportable conduct investigations across education, NDIS, private sector and state government clients.

Tanya Rosemond-Stephens
Tanya is an experienced investigator and safeguarding specialist with more than 30 years of experience across child protection, community services, law enforcement and organisational leadership. Her career includes senior roles with the NSW Police Force, the NSW Department of Communities and Justice, CatholicCare Social Services and Samaritans Foundation. Tanya has extensive experience conducting and overseeing reportable conduct, workplace grievance, code of conduct and child protection investigations, and brings a strong understanding of procedural fairness, risk assessment and child-safe practice to every matter she takes on.

Daniel Kensey
Daniel is a workplace relations specialist and investigator with more than 20 years of experience across education, not-for-profit organisations, employer associations, government and trade unions. He has conducted and managed investigations involving misconduct, bullying, harassment, discrimination, reportable conduct and complex employee relations matters. Daniel holds a Master of Labour Law and Relations from the University of Sydney, where he received awards for academic excellence, and brings strong expertise in procedural fairness, workplace law, disciplinary processes and the resolution of sensitive workplace disputes.

Kyle Wilkins
Kyle is a workplace relations and investigations professional with more than 20 years of experience across complex, highly regulated organisations including Qantas, Guardian Australia, The Y NSW and ANSTO. He brings a calm, measured approach to sensitive and high-stakes matters, with a strong commitment to procedural fairness and defensible outcomes. His experience spans workplace misconduct, code of conduct, reportable conduct and systemic reviews, and he is valued by clients for his clear communication, sound judgement and ability to manage competing interests with discretion.

Chris Gomez
Chris brings 14 years of experience with Victoria Police, including seven years as a Detective investigating serious and complex offending. His policing background gives him a strong practical foundation in evidence gathering, witness management and building investigative briefs that hold up to external scrutiny — skills that translate directly into workplace misconduct and reportable conduct investigations. Chris has specialist training in reportable conduct investigations and understands both the regulatory requirements and the human dynamics that make these matters particularly complex to manage well.

At Insight Investigations we take a structured, legally defensible approach to every investigation. Our process is built around procedural fairness — ensuring that everyone involved is treated with fairness, consistency and respect throughout the process.
Our reports are thorough and designed to withstand scrutiny from regulators, tribunals and courts. We work closely with your organisation from the moment we are engaged, keeping you informed at each stage while managing the investigation independently and without interference. When the investigation is complete, you will receive a report that fully analyses the evidence and makes clear, defensible findings.
